Checklist for required DCOM file materials

 

 

 

1) Proper enrollment in GBHEM online candidacy application system by the District Superintendent

2) GBHEM Form 102 (Biographical Information)

3) GBHEM Form 103 (Medical Form)

4) GBHEM Form 104 (Declaration of Candidacy for Ministry)

5) Written response to statements in BOD, Paragraph 310.2a

6) Commitment to Highest Ideals of Christian Life form reviewed and signed, click here

7 )Documentation of graduation from high school or equivalent, or transcripts from undergraduate and/or graduate level studies as applicable.

8) Completion of Nurturing Wholeness training (provide copy of completion certificate). Check the Pen-Del website calendar for upcoming dates.

9) Completion of Basic Sexual Ethics training (provide copy of completion certificate). Check the Pen-Del website calendar for upcoming dates.

10) Completion of Orientation to Ministry group mentoring or Exploration of Ministry Guidebook with individual mentor. Date of completion should be noted in your mentor’s report to be included in your interview materials packet. Check the Pen-Del website calendar for upcoming dates.

11) Completed Criminal & Credit Background Check (paid for by candidate). Go to www.oxforddoc.com; the Pen-Del Conference number is 526 (enter when prompted). The position code is 1

12) Completed Conference-approved Psychological Evaluation (paid for by 1/3 candidate, 1/3 home church, 1/3 BOOM). After Forms 101-104 and a completed Credit and Criminal Background check have been received by the DCOM Registrar, candidate's name will be forwarded to the BOOM coordinator of Psychological Evaluations, who will then contact the candidate with upcoming dates available

NOTE: It is your responsibility to keep the Registrar updated with any changes to your email, mailing address or contact phone numbers.

After your initial interview with DCOM, you need to submit the following materials and complete the following tasks before consideration for certification as a ministry candidate.

These materials are separate from the required yearly interview materials listed on the Yearly Interview Materials page. Checklist items and interview materials are BOTH required.
